SYL Online Registration 2005

• Registrants who submit registration information electronically through our online registration page will not need to attend an informational meeting in order to sign their paper registration forms. Instead, we will deliver pre-filled forms to you by email in Adobe Acrobat^® format. You can print them out, sign them and mail them back with your check.

• There is so much extra work involved for us in handling non-electronic registration that, although we still will accept that method, we will charge an extra $5.00 for registrations processed in that way.

Benefits. Online registration will benefit you as an SYL registrant by allowing you to enter your registration information at a time that is convenient for you, and in a place where you have all your resources available. This should allow you to supply complete and accurate information. It also will save you from entering duplicate information on several separate forms. And online registration will benefit SYL by giving us more complete and more accurate information while saving us a great deal of data entry.

Privacy. When you use our online registration page, the information you enter will be transported directly to our database. Your information will not be shared with any other organization except US Lacrosse and their insurance carriers. This is consistent with our past practices.

Pre-Filled Fields. You will find that our form makes certain assumptions as you fill it out. Information that you enter in earlier fields sometimes will be duplicated for you in later fields. We believe that this will ease the data entry task for most of our registrants. If our method makes more work for you, we apologize.

Completeness. You also will find that our form requires almost all of the fields to be filled out before it will allow you to submit your registration. This is to encourage folks to give us complete information up front, so that we won’t have to take extra time later following up. But if you have important reasons for omitting information that we require, please enter dummy information in the relevant field and then explain what you have done in the ‘Miscellaneous Notes’ field at the end of the form. Please don’t use this method if you just need extra time to collect the information. In that case, it will be far more helpful to submit the form later, when you have been able to gather the missing data.
